If Dr. Brosmer is currently out-of-network (e.g., Magellan), one self-pays 2021 Indiana-based Medicare rates, which are discounted from our full fees.  These payments are due at time-of-service and are not applied toward a deductible unless the client chooses to submit the claim.  The cost for the initial visit is $135, which is $70 discount from our full fee of $205. Subsequent visits are $135, which is a $35 discount from our full fee of $170.  Payments can be made via cash, check, credit or HSA card.  All cards are assessed a 3% admin fee, so we encourage the use of cash or check.
